Diagnosis of cow milk allergy in the gut, never an easy task.

O artigo de Lins et al.1 é um estudo importante porque ressalta vários fatores-chave da alergia à proteína do leite de vaca. Os pesquisadores identificaram, com base na história, 66 pacientes com reações adversas à proteína do leite de vaca. Essas reações incluíam uma variedade de sintomas cutâneos, gastrointestinais e sistêmicos. Mediram-se os níveis de imunoglobulina E (IgE) sérica total e específica, incluindo anticaseína, anti β-lactoglobulina e anti α-lactalbumina. Os pacientes foram orientados a seguir uma dieta de exclusão do leite de vaca por pelo menos 2 semanas, utilizando fórmula de proteína de soja. Após a resolução dos sintomas, um teste de desafio com proteína do leite de vaca foi realizado em ambiente hospitalar, com consultas semanais de acompanhamento por 4 semanas consecutivas após o teste, período durante o qual os pacientes continuaram a consumir leite de vaca. Aproximadamente metade dos pacientes não apresentou reação positiva à proteína do leite de vaca. Naqueles que tiveram reação positiva, uma reação tardia foi comum, ocorrendo em 77% dos casos. Os pacientes apresentaram reação até a terceira semana de consumo de proteína do leite de vaca. Os níveis de IgE específica de duas subfrações de proteína de leite de vaca foram elevados em somente uma porcentagem muito pequena dos pacientes com resultado de teste positivo.
